Visitor COVID Vaccination Policy & Process
From Friday 17th December 2021 the QLD Health Vaccination Directive for visitors to healthcare providers will come into effect. As a paediatric healthcare setting* the directive for Active Eight is clear:
No child or young person or their parent, carer, or guardian will be denied access to care or treatment based on their vaccination status.
We recognise that children and young people generally present to healthcare services with a parent, carer, or guardian and will ensure that family support is continued in a COVID-safe way.
There will be some restrictions for unvaccinated visitors including members & siblings over 16 years of age, unvaccinated parents, carers, and guardians accompanying a child or young person to Active Eight. These restrictions include:
Only one unvaccinated parent, carer, or guardian can accompany a child or young person to an individual therapy appointment or group program;
An accompanying unvaccinated parent, carer, or guardian must always wear a single-use surgical face mask;
An accompanying unvaccinated parent, carer, or guardian must limit their movements whilst in the healthcare setting and maintain physical distancing (1.5 meters) at all times. This means unvaccinated parents are to remain seated in the waiting room upon entry and are not to enter the therapy room unless an exceptional circumstance may apply (your therapist or an Active Eight staff member will notify you if an exceptional circumstance applies);
There may be situations that require special consideration which will be worked through on a case-by-case basis as required. This includes an exemption request for a second unvaccinated parent, carer, guardian, or sibling over the age of 16 to attend.
These restrictions will not apply to:
Active Eight members under the age of 16
vaccinated parents, carers, guardians, members, or siblings over the age of 16
visitors and/or siblings under 16 years of age
Entry for ALL VISITORS into an Active Eight facility will only be granted if the entry requirements have been met. These entry requirements are:
The above Visitor Vaccination Check-In and Check-In QLD QR Code must be completed at the front door or prior to entry. An Active Eight team member will kindly ask if these check-ins have been completed when you enter the facility and you may be asked for confirmation. For ease, a QR code linking to this form will be provided alongside the QLD Check-In QR code.
Single-use surgical face masks are strongly encouraged to be worn by all visitors whilst at Active Eight, however, masks will be mandatory for unvaccinated visitors over the age of 16.

Update: QLD COVID-19 Mask Mandate
From 1am AEST Sunday 2 January masks must be worn indoors everywhere in Queensland, including healthcare settings*. You do not need to wear a where it is unsafe to do so, such as while doing strenuous exercise.

*Healthcare setting means a setting or premises where healthcare is ordinarily provided by a healthcare professional including; outreach services in other settings provided by private healthcare facilities, including in-home healthcare services; or disability support services delivered in the home and school based healthcare, including in special schools.
